Just received my first lyophilized semaglutide vial (5mg) and a vial of bacteriostatic water and I'm staring at them like they're a bomb I need to defuse. 😂

Can someone walk me through reconstitution step by step? I've watched YouTube videos but they all seem to do it slightly differently and I want to make sure I get this right.

My vial says 5mg lyophilized semaglutide. I want to dose at 0.25mg per week to start. How much bac water do I add?

Welcome to the club! It's way less scary than it looks. Here's the math first, then the procedure:

RECONSTITUTION MATH:

You want to make dosing convenient. The standard approach is to add enough bac water so each "unit" on your insulin syringe equals a clean number.

If you add 2mL (200 units) of bac water to a 5mg vial:

  • Concentration = 5mg / 2mL = 2.5mg/mL
  • Each "unit" on a U-100 insulin syringe = 0.025mg
  • For a 0.25mg dose: draw to the 10 unit mark
  • For a 0.5mg dose: draw to the 20 unit mark
  • For 1.0mg dose: draw to the 40 unit mark

Nice clean numbers. I always use 2mL for 5mg vials for this reason.

STEP-BY-STEP PROCEDURE:

  1. Wash hands thoroughly with soap and water. Seriously, don't skip this.
  2. Gather supplies: alcohol swabs, 1x 3mL syringe with needle (for reconstitution), your lyo vial, bac water vial, and your insulin syringe (for injection later)
  3. Swab both vial stoppers with alcohol pads. Let dry 30 seconds.
  4. Draw 2mL of bac water using the 3mL syringe. Pull back the plunger to 2mL, insert needle into bac water vial (inverted), inject air first, then draw out 2mL.
  5. INSERT NEEDLE INTO LYO VIAL — aim the needle at the glass wall, NOT directly at the powder cake. This is critical.
  6. Inject the bac water SLOWLY — let it drip down the side of the vial. Do NOT squirt it directly onto the powder. Peptides are fragile — aggressive reconstitution can damage them.
  7. GENTLY swirl — do NOT shake. Tilt the vial at 45° and rotate. The powder should dissolve within 1-3 minutes. If it doesn't fully dissolve, set it down and wait. Some peptides take up to 10 minutes.
  8. Solution should be clear and colorless. If it's cloudy or has particles, something went wrong. Do not use.
  9. Store refrigerated at 2-8°C. Use within the beyond-use date (typically 28-30 days for bac water reconstituted peptides).

This is EXACTLY what I needed. Two questions:

  1. Can I use regular sterile water instead of bacteriostatic water?
  2. What's the deal with aiming at the glass wall? What happens if I squirt it directly on the powder?

1. NO. Regular sterile water does not contain the 0.9% benzyl alcohol preservative. Without that preservative, bacteria can grow after you puncture the stopper. Sterile water is for single-use only — if you're drawing from the vial multiple times over weeks, you NEED bacteriostatic water. This is non-negotiable for multi-dose vials.

2. Direct impact on the lyophilized cake can cause foaming and protein denaturation. You're essentially hitting delicate peptide molecules with a high-pressure stream. It won't necessarily ruin it, but it can reduce potency. The gentle approach preserves the peptide structure. Think of it like... don't pressure-wash a silk dress. 😄
