The early improvement Michelle describes is consistent with the hypothesis that some of tirz's OSA benefit is weight-independent. Fat deposition in the parapharyngeal space (around the throat) may redistribute or reduce earlier than total body weight change would predict, possibly due to GIP-mediated effects on visceral/ectopic fat deposits.

There's also emerging data on GLP-1 receptor expression in the brainstem respiratory centers (nucleus tractus solitarius) that could influence respiratory drive during sleep. This is very preliminary but mechanistically plausible.

Thank you all. I'm on 7.5mg currently, 3 months in, down 28 lbs. My wife says the snoring is "significantly better" but not gone. I've got a repeat sleep study scheduled for month 6. Fingers crossed my numbers look like Michelle's. 🤞

Dr. Martinez — thank you for the responsible reminder about not stopping CPAP without proper testing. As much as I hate the machine, I'll wait for data before making any changes.

12 years of CPAP, maybe not 13

Brian — 28 lbs in 3 months with subjective snoring improvement is very encouraging. I'd expect your AHI to have improved meaningfully already. The month 6 study will give you the data you need.

And yes — patience with the CPAP. I know it's annoying but unmonitored discontinuation can be dangerous, especially if there's residual moderate-severe OSA. The repeat study will tell you if it's safe to stop.

I'm genuinely excited about where this field is going. The combination of effective weight loss medications and improved PAP technology means we have more tools than ever for OSA management. Good times for our patients. 👍
