Ok this is gonna sound weird but has anyone else experienced a SURGE in energy on tirzepatide? I'm on 5mg (week 6) and I feel like I drank four espressos. Not jittery exactly, just... awake? Alert? I'm voluntarily exercising at 6am which is something that has NEVER happened in my 42 years on this planet.

My husband thinks I'm manic. I'm not manic. I just have... energy? Is this what normal-weight people feel like all the time?? 😂

Is this a known thing or am I just weird?

Not weird AT ALL. I experienced the exact same thing starting around week 4-5 on tirz. I described it to my wife as "someone turned the lights on in my brain." I went from being a couch potato to walking 8,000 steps a day without even trying.

I think part of it is just... eating less garbage? Like when you're not consuming 3,000 calories of processed food, your body just runs better. But there might be a pharmacological component too.

This is a commonly reported phenomenon and there are several potential explanations:

  1. Improved insulin sensitivity: Tirzepatide dramatically reduces insulin resistance. Hyperinsulinemia is associated with fatigue and brain fog. As insulin levels normalize, many patients report improved mental clarity and energy.
  2. GIP receptor effects: Unlike pure GLP-1 agonists, GIP has receptors in the CNS that may modulate energy expenditure and perceived energy. This is still being researched but early data from rodent models suggests GIP signaling in the hypothalamus affects activity levels.
  3. Reduced systemic inflammation: Excess adiposity drives chronic low-grade inflammation (elevated IL-6, TNF-α, CRP). As patients lose weight and visceral fat decreases, inflammatory markers drop, and patients often report feeling "better" in a general sense.
  4. Better sleep: Weight loss (even 5-10%) can dramatically improve sleep quality, particularly if there was undiagnosed or subclinical sleep apnea. Better sleep = more daytime energy.
  5. Dietary quality: As mentioned above — many patients naturally shift toward higher-quality foods because the volume reduction on tirz makes them more selective. Better nutrition → better energy.

If the energy feels excessive, disruptive to sleep, or is accompanied by racing thoughts or impulsive behavior, mention it to your prescriber to rule out other causes. But what you're describing sounds like a positive therapeutic response.

Dr Patel you're a gem as always. I think #4 might be a big factor — my husband says I've stopped snoring for the first time in years. Probably was mild sleep apnea that's resolving with the weight loss.

Also #5 — I ate a salad for LUNCH today. By CHOICE. Who even am I anymore? 🥗

Opposite experience here 🙋‍♂️ Tirz makes me exhausted for about 48 hours after injection, then I feel normal-to-good the rest of the week. My energy definitely didn't increase overall. I think it's very individual.

That said I'm also on a bigger caloric deficit than most people here (probably eating 1,200 cal/day which I know is too low, working on it).
